Prior to assessment and treatment of your animal, please read the following terms and conditions in full (see below). If you accept these terms and conditions, please sign by entering your name, your contact details and the date, and clicking submit, thank you.
Terms and Conditions
General
Animals will be seen for physiotherapy in accordance with the Veterinary Surgeon’s Act 1966 and Exemptions Order 2015. Any physiotherapy assessment and/or treatment of your animal(s) may be discussed with your veterinary surgeon as required, and I may refer an animal back to their veterinary surgeon for further investigation if deemed necessary at any stage.
We reserve the right to refuse treatment if it is deemed to be either inappropriate or unsafe (such circumstances might for example include the presence of undiagnosed lameness, infectious or contagious diseases, or other health or behavioural concerns). We do kindly request that you always let me know in advance if there are any changes to the health of the animal due to be seen between the time of booking and their appointment.
All animals need to be dry and brushed clean with visible mud removed for effective assessment and treatment to take place.
Please cancel your appointment if your animal is suffering from a contagious disease (you will not be subjected to the Cancellation Policy as detailed below in this circumstance).
Payment
Charges are as follows for initial and follow-up appointments:
Equine £65 (up to 90 minutes)
Canine £50 (up to 60 minutes)
Payment is to be made in full by cash or BACS on the day of treatment. Receipts are available upon request.
Insurance claims are to be made by the clients themselves as we do not deal directly with insurance companies. Payment must be made in full, and clients are responsible for claiming back the costs from their insurers themselves.
Cancellation Policy
Please give 48 hours’ notice of cancellation where possible, to allow the appointment to be available for booking. Less than 24 hours’ notice will require full payment; this is subject to circumstances so please discuss individual cases with us.
Service Provision
We reserve the right to refuse services to anyone at any time.
While every effort is made to ensure the safety of the animal and riders/handlers during sessions, we accept no liability for any loss or injury occurred during the session. We ask that owners/handlers provide a safe environment to work in and that handlers/riders are competent.
Sometimes photographs and videos are taken during sessions which may be used in social media posts and for marketing purposes. If you do not wish for you and/or your animal’s image to be used in this way, please let us know.
To ensure high standards of treatment and care we will:
Explain to you how physiotherapy can help your animal, including the benefits and risks associated with treatment.
Undertake a thorough assessment prior to commencing any treatment and explain the results of this assessment to you.
Explain the treatment to be provided during each physiotherapy session.
Maintain contact with your veterinary surgeon during the course of treatment if the animal has been referred.
Contact your veterinarian with any concerns of underlying injury, disease or pathology after discussing our findings with you if the animal has been seen for musculoskeletal maintenance.
Upon completion of treatment appropriate management advice will be provided. A written discharge summary will be sent to you, and to your veterinary surgeon if the animal has been referred.
What we expect from you:
Please notify us of any change in medical condition to you/your animal.
If at any time you are unhappy with your treatment, you should inform us immediately to see if the matter can be resolved informally. You are entitled to make a complaint. Complaints are treated seriously, and your complaint will be dealt with promptly and professionally in accordance with our Complaints Policy.
You can view your treatment record at any time, please contact us to discuss this.
You can refuse treatment for your animal at any time.
I, the owner of the animal above, hereby give my consent for physiotherapy assessment and a course of treatment of the above animal, and give my consent for the physiotherapist to contact my veterinarian with any concerns that the animal may be presenting an underlying injury, disease or pathology.
